Wellness Pathways

Wellness Pathways is a Medicaid-approved HCBS provider dedicated to delivering compassionate, person-centered care that supports individuals in living with dignity, independence, and purpose. We serve Aurora, Colorado Springs, and surrounding communities, offering homemaker services, supported community connections, and Individual Residential Services & Supports (IRSS/Host Home). Our approach is rooted in respect, consistency, and building meaningful relationships with those we serve. As an RN-owned agency, we take a hands-on and quality-focused approach to care. What sets us apart is our responsiveness, flexibility, and commitment to starting services quickly, typically within 5–7 days of referral. We strive to create a supportive and inclusive environment for individuals of all backgrounds, ensuring each person receives care tailored to their unique needs while maintaining the highest standards of professionalism and accountability.
