The mission of Six Points is to enhance the lives of the people we serve so they may reach their highest potential.
The purpose of Six Points is to provide advocacy, assistance, community, education, integration, and training to the people we serve in Gunnison and Hinsdale Counties of Colorado to enhance their well-being, personal dignity, independence, and vocational opportunities. Programming at Six Points includes: (A.) Residential Comprehensive Services for clients who live in housing owned or rented by Six Points, including: • personal care - eating/drinking, personal hygiene, emergency response, medications, supervision, health monitoring, and positioning/transferring • household assistance - money management and disbursements, shopping assistance, laundry, meal preparation, household cleaning and maintenance • mentoring - decision making, planning, appointment scheduling, safety, health, dietary assessment, relationship education and support • transportation - accessing community services, employment, appointments, shopping, banking, civic events or social activities; and • community accessibility/connections - acquisition of community assistance, recreational/community activities, adaptive skills, socialization, and supplies. (B) Supported Living Services for clients who live in their own homes or with family members (offering the same services as above for Residential Comprehensive Service, on an as-needed basis, and including much of the Vocational/Day Programming listed below); (C) Supported Employment/Day Programming/Division of Vocational Rehabilitation (DVR) Services which provides job services and day programming for clients and other individuals contracted with DVR including: • employment assessment • life skills training • individualized employment support • job placement assistance • facilitating strong relationships between clients and community employers • identifying and requesting reasonable accommodations • supported community connections; and (D) Thrift Store operations serving the entire Gunnison Valley as the largest thrift store in the area. The Thrift Store provides employment for many of our clients and volunteer opportunities for community members. The Thrift Store provides an outlet for residents to donate gently used goods, reducing waste, which helps sustain our natural resources and environment. It also provides a crucial source of affordable merchandise for low-income populations of Gunnison Valley, as well as for several other agencies and vulnerable populations that Six Points supports with donated merchandise.
