Passionate Care Services LLC provides community-based services through Individual Residential Services & Supports, Supported Community Connections, and Non-Medical Transportation. Passionate Care Services LLC will work with each person to design and lead their own person-centered plan to maximize independence and maintain health and safety. Services and supports will vary according to the person’s own plan and support needs, but may include the following: • Community Integration • Skill Development • Socialization • Healthy Living • Activities of Daily Living • Education and Self-Advocacy • Medication Administration • Money Management Skills • Behavioral Support • Transportation • Personal Care/Homemaker Services
