Welcome to the Journey of Care family. What Sets Journey of Care Apart? At Journey of Care, Remote Support isn't just another service we offer—it's our specialty. It's all we do. While state requirements focus primarily on scheduled check-ins and responding when members reach out, our model is intentionally built to go far beyond those minimum expectations. Every member is supported by a skilled team of Care Coordinators whose job is not simply to monitor safety, but to actively help members navigate the healthcare system, remain independent, and solve the everyday challenges that can threaten their health and well-being. Our team routinely assists with scheduling and coordinating physician appointments, arranging transportation, following up after hospitalizations, helping members understand treatment plans, communicating with providers, resolving medication concerns, connecting members with community resources, and working alongside families and caregivers. In select communities, we also collaborate with trusted partners providing in-home nursing, primary care, physical therapy, behavioral health, and other supportive services to help ensure members receive the right care at the right time. This level of ongoing care coordination is what truly sets Journey of Care apart. While many Remote Support programs focus primarily on technology or scheduled wellness checks, we focus on relationships. We get to know our members, recognize when something has changed, and take action before small problems become crises. Members can reach us through a tablet, phone call, or text message—whichever works best for them. Behind every interaction is a real person who knows their story, understands their goals, and is committed to helping them succeed. We also believe Remote Support should strengthen—not replace—the care members already receive. Our services complement in-home caregivers, case managers, physicians, therapists, family members, and other providers by helping everyone stay connected and informed between visits. Our goal is simple: help people stay healthier, safer, more connected, and more independent at home through exceptional Remote Support and proactive care coordination. Going above and beyond isn't an extra service. It's simply how we deliver care. It's all we do.
