Evergreen Service Providers is a small, but very experienced, non-profit organization dedicated to supporting individuals with intellectual and developmental disabilities to thrive and succeed! We provide Supported Community Connections (SCC) and Individual Residential Services and Supports (IRSS) through Host Homes and Family Caregiver settings. We pride ourselves on cultivating and maintaining close relationships with the individuals, family caregivers, and host homes we support. We work closely with our family caregivers and host homes through 24/7 on-call support, nursing consultation and oversight, monthly home visits and person-centered planning.
