Across the state through locations on the Front Range and Western Slope, Mosaic serves nearly 400 people who have chosen our highly personalized home- and community-based services that let those we serve decide where, with whom and how they live. Our mission is to empower adults and children with intellectual and developmental disabilities, mental and behavioral needs and autism to live their best life. We work to help people be as independent as possible by teaching life skills such as healthy eating, money management, safety and more. In addition to our team of IDD professionals, Mosaic provides all caregivers and individuals served access to a 24-hour nurse hotline to help get the health information you need, when you need it. Whether a person wants to live independently in their own home, in a family caregiver setting, or with a host home provider, Mosaic offers support that’s right for you. We provide supports through the Developmental Disabilities waiver, the Supported Living Services waiver, the Children’s Extensive Services waiver, and the Children’s Habilitation Residential Program waiver, and Community First Choice.
