We offer small-group community connections and day program services, Family Caregiver services for residential and day program, and contractor SCC services. Programs are designed around daily growth, community involvement, and meaningful experiences.
~We offer small-group sizes for our Supported Community Connections program and Specialized Habilitation program (SCC serves 2-4 individuals per group and SH is 3-6 individuals per group). We provide all transportation and are open 5 days a week. The community outings and field trips are paid by our program so we can offer a wide variety of activities. ~Our small groups allow individuals to build friendships, stay engaged in the community, and participate in both fun and educational activities without getting lost in a large group. ~We also support one-on-one SCC services provided by host home providers and family caregivers. We do our best to make this as simple for the caretakers and individuals as possible to allow for the best community experience that fits each individual's unique needs. Not every individual wants to be in a group and we understand and support that. ~Our Family Caregiver program supports family members and caregivers who would like to provide Residential or SCC services to their loved ones. We understand and respect that you know your family member best, so we are here to guide you through unique situations, simplify state requirements, and ensure the process is as seamless, supportive and enjoyable as possible for everyone involved.
